How We Support Your Loved Ones

Due to every individual being unique, Acorns tailor the staffing teams, daily routines and environments to specifically suit the individual’s care and support needs.

What we offer:

● Individualised care and support plans

● Skilled and competent staff trained in all neurological and psychological specialisms that are integral to their understanding of learning disabilities, autism, and complex mental health conditions

● All staff are trained in trauma-informed care

● Homes that are safe, comfortable, and adapted to suit physical and psychological health needs

● Daily meaningful activity with encouragement of community involvement including volunteering, education, hobbies, sport, activities of interest

We meet your loved one and begin crafting a care and support plan built around their needs, goals, and preferences.

We encourage as much input as possible from the individual, and we are experienced in a variety of communicative methods, should an individual require an alternative method of communication.

Step 3: Find the Right Home

We help secure a suitable property through trusted housing partners.

Step 4: Begin the Journey

We’ll be there every step of the way to ensure a smooth transition takes place and ongoing support continues for your loved one.

What is supported living?

Supported living is where an individual lives in their own home (or shared home tenancy) with support that’s personalised to them. Supported living is more flexible, independent and empowering than traditional residential care.

Where are your homes?

Our homes are located in Hampshire.

Is supported living suitable for individuals with complex needs?

The staffing teams at Acorns are extremely experienced in working with complex health needs and behaviours that challenge. The staff have continuous training and are supported robustly by management. Acorns take pride in the fact that all management and senior management have started their careers within Social Care working frontline with complex needs and behaviours that challenge.

How will my loved one be cared for?

We provide nurturing, warm and stable environments where individuals feel respected, secure and at ease. Our experienced teams deliver consistent, thoughtful and compassionate support tailored to each individual’s unique needs, wishes and circumstances.

Can I still be involved in their care?

Yes, absolutely. You will be a part of the Acorns team should your loved one wish you to be. You have access to our care planning system whereby you can see everything your loved one is achieving throughout the day. Acorns involve families from the assessment stage, and throughout the ongoing care and support.

You will have open communication with the frontline staffing team and management. You will also have access to Care Control, which is our internal system for recording and reporting daily activities, and any cause for concern. This is naturally dependent on whether you are provided permissions to access Care Control.

What about funding?

Most care and support is funded by the Local Authority or the Intergrated Care Board. Your Social Worker, or overseeing professional, will support you in applying for the correct funding and ensure that your care and support needs are funded appropriately. You may be eligible for Housing Benefit which can be applied for.
