Make a referral
Regardless of the origin of a referral, we apply the same comprehensive needs-led assessment to ensure that we understand the individual’s needs and aspirations.
This process includes meeting with the individual, their family and any other stakeholders in their life.
Where both we and all interested parties conclude that we can meet an individual’s needs, we then put together a detailed transition plan, in full consultation with all parties, to ensure that the placement is undertaken in the best way possible for the individual.
