Our Approach to Care

At Acorns Health Care, we deliver highly personalised care and support for individuals diagnosed with learning disabilities, autism, and complex mental health conditions.
Acorns recognise everyone is individual and unique. Our model of care centres around understanding an individual’s history, goals, strengths, alongside clinical needs, by ensuring support is not only safe but meaningful and progressive.
Key principles of our approach:
● Individual-led care and support that prioritises autonomy, dignity, and self-determination
● Risk-aware, not risk-averse. Encouraging positive risk-taking for personal growth and achievements
● Co-produced support planning involving the individual, family, and relevant professionals
● Trauma-informed and neurodiverse-affirming practices

We provide a range of settings, including solo tenancies and shared environments, supported by fully trained and compassionate staff. Acorns are proud to concentrate on least restrictive practice, working closely with the surrounding professionals and clinicians, in ensuring the support and care is safe.
Acorns use the physical intervention training delivered by Loddon called PROACT-SCIPr-UK and a robust clinical governance structure.

Referral Process and Criteria
We welcome referrals from social workers, commissioners, care coordinators, community mental health teams, and other professionals supporting individuals in need of tailored care.

Who We Support:
● Individuals aged 18+
● Diagnosed with learning disabilities, autism, or mental health conditions
● Individuals that might present with behaviours that challenge
● May be transitioning from hospital, residential settings, or home settings
● Eligible for supported living funding via their local authority and/or ICB
Referral Steps:

Step 1: Let's Talk
Call or send us a message to share your initial enquiries about an individual you are currently overseeing. We’ll listen and offer guidance and support, with no obligation for next steps to be explored if we are not the right provider.

Step 2: Assessment
We meet the individual you have referred to us and begin crafting a care and support plan built around their needs, goals, and preferences.
We encourage as much input as possible from the individual, and we are experienced in a variety of communicative methods, should an individual require an alternative method of communication.

Step 3: Find the Right Home
We help secure a suitable property through trusted housing partners.

Step 4: Begin the Journey
We’ll be there every step of the way to ensure a smooth transition takes place and ongoing support continues for the individual.
We will work closely with all surrounding professionals to ensure the transition is as effective and robust as possible.
